RN Care Manager - Seattle, WA This is a HYBRID role (2 days/wk on site, 3 days/wk remote), open to LOCAL and NON-LOCAL candidates, - Position / Specialty: RN Care Manager - Schedule: 5x8hr, M-F, 080-1600 (2 days/week on-site at Beacon Hill clinic, 3 days/week working from home. SPECIAL NOTES - Candidate is expected to have their own reliable transportation (NOT public transportation) for the duration of this contract. - This Care management role focuses on transitions of care, including care coordination and medication reconciliation, with the goal of reducing ED visits and readmissions. - All IP discharges, with the exception of labor and delivery and oncology follow-up. - All ED visits, particular focus on patients with chronic conditions.
